School Physicals (Florida DH 3040)

Florida law requires a School Entry Health Exam (Form DH 3040) for every student entering kindergarten and for any student new to a Florida school district. The form must be signed by a licensed healthcare provider and dated within the 12 months before the first day of school.

What’s included

  • Review of medical history and immunization records
  • Height, weight, BMI, and blood pressure
  • Vision and hearing screening
  • Heart, lung, abdominal, and general exam
  • Developmental and wellness screening
  • Completed and signed DH 3040 form

What to bring

  • The blank DH 3040 form (we keep copies on hand if you forget)
  • Your child’s shot record — we can update immunizations during the same visit if anything is missing
  • Any medical records from a previous pediatrician or family doctor

Sports Physicals (FHSAA EL2 / EL3)

Every middle-school and high-school athlete in Florida must complete the FHSAA Preparticipation Physical Evaluation annually before tryouts or practice. The form has two parts:

  • EL2 — the medical history form (completed by parent/guardian and student)
  • EL3 — the physical examination form (completed by the provider)

Both must be signed and dated within 365 days of the athlete’s first practice of the season.

  • Height, weight, vision check, blood pressure, pulse
  • Heart, lung, joint, and musculoskeletal exam
  • Concussion history and recovery status
  • Review of any prior injuries or surgeries

Tip: Fill out the parent and athlete history section at home before the visit. It speeds things up considerably — and occasionally surfaces a family history detail worth discussing with the provider.

If your child needs both a school physical and a sports physical, we can complete both during the same appointment. The two exams overlap heavily, and there’s no reason to make two trips. Just bring both forms.

Heads up on timing: The weeks before school starts (late July through early August) and right before fall, winter, and spring sports seasons are our busiest. Walking in early in the morning, or booking ahead, will get you in and out faster.
